Good Morning,

Can someone please help me? I am trying to bill 64450 from an ASC with a mod 50, but Medicare is denying it. Should it be 64450 -50 or 64450 -RT & 64450 -LT?
 
What are they denying it for?

You might want to check your Medicare LCD's. Medicare will not pay for 64450 with certain diagnosis codes that are related to systemic diseases. If the injection is not related to a systemic disease you will need to appeal with medical records.
