Does anyone know of Medicare's policy for electronically signed EKG's by the Physician if they can just be signed on the EKG system such as midmark and only stored in midmark as documentation or if they also have to be included and documented in the EMR system and the Patient's chart? Anyone have any documentation on their requirements? Our Physicians are asking. Thanks for any help.

Hi! It would certainly be best practice to have the ECG included in the EMR and the patient's chart. CPT states that the ECG must have a separate signed, written and retrievable report. So a signed tracing in the midmark system alone is not considered sufficient documentation. Having said that, I think it would be easiest for the providers to have the ECG in the chart. Do they really want to have to access midmark separately and retrieve the tracing every time they want to review one?
I hope that helps. :)
