I wanted to make sure that the information that is being provided to me at work, is correct before I code something incorrectly.


**If a Medication level is ordered, such as Digoxin, Valproic acid, Keppra, etc… you should code these to the reason the patient is taking the med. For example Digoxin is used to treat heart failure or AFIB. So the best DX would be I50.9, or I48.91. If any Medication level is ordered and no DX is provided or you do not see a relevant DX on file, the best code to use is Z79.899 for the medication use itself. **

It depends on the reason for ordering the level. Nif it is just to check the level in an otherwise asymptomatic patient, then you use the Z51.81 for lerapeutic drug level monitoring with the Z79 COD for the drug secondary. If it is due to over dose or under dosing then follow the guidelines for those scenarios.
