Good morning, I am not sure if this is the best place to post this question or not... For Humana Medicare advantage risk plan, are patients in Long term care excluded from quality/HEDIS measures? Specifically for med rec post d/c? The only thing I am seeing in the guidelines for exclusion are patients in Hospice or using Hospice services. I appreciate any input.
