I work for an FQHC in Oregon. Is someone able to refresh my memory on how often an assessment is required to bill for services? My notes state that an assessment must be updated every 90 days (not necessarily billed) or when the patient's primary diagnosis for mental health services change. Other than that the only requirement I can find is to bill an assessment annually.
