When using the UB04 form to bill for Mental Health services and we are billing for group therapy, does the therapist go on the claim or the supervising provider (medical director) in this case? If the supervision provider goes on the claim, is a HO modifier used?
We are being told that the medical director is the only person that should appear on the claim, but he isn't the one that facilitates group therapy, so I am confused.
