If a doctor writes metatisis neoplasm do you code the secondary diagnosis ? Just wanted to make sure i was doing it correctly. Thanks

Unless the MD documents metastasis to bone, for example, I would code as secondary 198.5
But if the MD states metastasis colon cancer then I would code 153.8

It does depend on how it is documented as to whether you code a primary neoplasm with an unknown secondary or a secondary with an unknown primary. Just remember that when you code a secondary site you must always code the primary site as well which can be active, hx of , or unknown.
