If a primary OB refers a patient to us for DM and we do a consult code and continue to follow this patient along with the primary OB and then at a follow up apt and Ultra Sound we find a heart defect or can we do another consult code for the the new problem since we will be consulting the primary OB on this as well??

The short answer is "NO" ... if YOU found the heart defect, the OB did not ask for your advice/opinion on this. So it's not a consultation.

You may legitimatelly have a higher-level established patient visit, but no consult.
