Wiki Microscope Used During Foreign Object Removal

When an OP report states ...with microcope in place.... does that mean an operating microscope was used and I should use 69990? Microsurgery was not stated in the OP report and it seems the microscope was only used for better visualization.

I do not bill for the 69990 because is usually included in the main procedure; the only carrier that I have seen that pays for it is medicaid when a microscope is used to insert tubes...hope this helps!
