My Dr did a Minimally Invasive Direct off-pump Coronary Artery Bypass Grafting x 1 using a single arterial graft and short segment vein jump graft (Lima-svg-lad via a left anterior mini-thoracotomy)

he connected the artery and vein together to use for the graft bc the LIMA was not long enough. so it is only 1 graft.
How would you code this?
