Our doctor is going to do a mini-laparotomy on patient with unilateral ovarian cystectomy and removal of a uterine mass. We need authorization so we have to send something over just not sure what.

58925 for your ovarian cystectomy. For the "removal of uterine mass", it may depend on what that mass is. If it's a fibroid, I would use 58140. If it's not a fibroid, I would be stumped and might have to go unlisted 58999. If it were me and this is just for precert purposes, I would just use the 58925. Or 58925 and 58140 if I were being overly cautious. I can't recall ever having an issue if we needed to perform additional procedures that were not precert. Wait, maybe once with United Healthcare Community Medicaid.
