Hi ENT coder/ biller here - My practice manager want to do High blood pressure reading as a recordable measure . I wondering how to code the diagnosis for high blood pressure patients if we are not treating the condition and would only be on past medical history . I am thinking may not be appropriate to code since we only have a blood pressure that was high that day and past medical history. I do see the code for a patient presenting with a High reading and no diagnosis. Any input would be appreciated.
 
Is doesn't matter if you are treating current high blood pressure. It is a chronic condition and would be coded if on current meds.
If it was just elevated BP reading w/o dx then code that R03.0
If they have a hx of HBP and not currently taking med it would be Z86.79
family hx Z82.49
