Wiki Mirena insertion?

We have a provider that inserted a Mirena but after inserting during that same visit the provider noticed that the strings had came out so he had to remove it and then could not reuse it. That same day he inserted another Mirena. How are we to bill for this would we use a JW modifier on the mirena that was wasted?

JW is not a valid modifier for J7298. Generally when this occurs, we will work with our distributer to have the item replaced/traded as the strings came off during insertion. It is not something that we bill to the patient or carrier.
