I had this come across my desk and this is very unusual that the Dr will address more than an annual wellness and diagnosis specific E&M on the same visit. In addition to a Medicare Wellness Exam G0439 and 99213-25 the doctor also used liquid nitrogen to take off a lesion (Dr used dx 782.2) so 17110 was billed in addition.
Insurance denied the wellness exam G0439 for reason "included in surgical package global" so question is... did I miss a modifier?

.. in addition I must add that the E&M diagnosis used were absolutely unrelated to the annual wellness exam or 782.2 (localized superficial swelling, mass, or lump)
