How would I charge for mitomycin in the HCPCS BOOK it says 5mg, but this procedure patient had only .04 mg, how do I charge this ?

you would have to bill a count of one, based on the billing increment. I don't know if it is a MDV or SDV. If SDV, waste should be documented.
