The definition of J9281 is Mitomycin pyelocalyceal instillation, 1 mg. If a provider instills 80 mgs. into the bladder, then would 80 units be correct? The vial that we purchase contains Mitomycin SDV 40 MG/VL 100ML/VL. So if there are 40 mgs per vial, then the quantity to bill may be only 2 units? I'm confused.

80 units is correct if the code description is 1 mg and an 80 mg dose was given. (If the code description was 40 mg, then you'd code 2 units for an 80 mg dose.) Vial size isn't relevant unless the code description happened to say 'per vial', but I haven't encountered any codes that use that kind of measurement.
