I know we have to add Mod 22 on primary codes that apply but for add on CPT codes for spinal fusions or laminectomies do we add Mod 22 on add on or just the main codes?
 
I have only ever had reason to apply -22 to the primary procedure. -22 indicates there was increased work, and does not necessarily specify how much additional work. For example, on some claim with -22, I ask carrier for 20% additional payment. On others, I might ask for 40% additional payment. Assuming my contract doesn't have a set specified amount.
Per this article https://community.auanet.org/blogs/policy-brief/2018/04/05/modifier-22-what-you-should-know, the -22 can be applied to any code. I would just make sure the documentation makes clear the extra work that was required.
