Can a modifier 22 be put on a complicated CPT code like 52318?

Absolutely, the 22 modifier is really telling the insurance you did more than usual. If you go to your final rule and find "physician time" this will show you how much time is needed for that procedure. If the provider spent way more time and effort on that procedure, then it would warrant the 22 modifiers. Make sure that extra time and effort is in the documentation so when you appeal for that extra reimbursement the documentation will show that.
