Hi, My question is if pt is admitted and requires a surgery that has a 90 day global and they also do a procedure in the same surgical session with a 10 day global would you need a 57 mod and a 25 mod for the E/M? Thanks!
 
I have never come across a situation like that however the 57 would be appropriate if the 90-day surgical procedure was taking place the following day. If it was then I would use both modifiers to cover both procedures however I could also see the E/M itself being denied if the purpose of the visit was for the the minor procedure. You would have to appeal to cover yourself for the "decision for surgery."
