Wiki Mod. 59 with Add on codes

I know that you do not use modifiers with add-on codes. The issue I am coming up against is insurance denials.
If I am billing out a 99203-25, 17110, 17000-59 and 17003 I am not adding a modifier 59 to the 17003. Should I be? Should the 17110 have a different modifier?

My experience has been that if you are only billing a single unit of the 17003 along with the other codes, then it will not need a modifier. Where it gets tricky and depending on payer, the use of modifier 59 or 76 (per our Noridian carrier) and/or units use may come into play. There has been much written on this in the forum swhen billing multiples of 17003. But as far as your original question is concerned. your placement of modifiers as to the way you have it is correct per my understanding..
