If I have a surgery which is done both sides and again on the left side at the same time, can I code like this: xxxxx-rt-lt, xxxxx-lt-59?

you can never use more than one anatomic modifier on one line. For bilateral depending on payer you can code
xxxxx-50 or
xxxxx-RT with
xxxxx-LT you do not need the 59 modifier.
