I am not sure what you are coding. Is this a home visit? If so, then you need to check your codes, as 99353 is no longer a valid code. The highest code for an established patient home visit is 99350. If this is what you are asking then no modifier is necessary as 99354 is an add-on code. However, if you are coding 99215 along with 99350 or another home visit code, then -25 is necessary.
