Hi, I'm billing at initial hospital care code 99223 with Thoracentesis with/imaging. The claim denied and it stated it can be overidded with a modifier. Would the modifier go on the initial hospital care code or the the thoacentesis?

Modifier 25 would be applied to e/m code 99223.
