If a Medicare Part B patient was seen for a regular EM vist for hypertension and diabetes, plus a screening pap smear, plus skin surgery with electrocautery and destruction for an neoplasm unknown, what would be the best scenerio for billing this claim? The doctor did all of these procedures on the same day due to transportation problems for the patient.

I agree but where did the 238.2 dx come from? This can be used only if the pathology indicates a neoplasm of uncertain behavior. I only ask because it is rare to see this documented by a physician and even more rare to perform a destruction for this dx.
