Wiki modifers and term definitions

Modifier 25 is used to signify that a separately identifiable E/M service was done on the same day that another procedure or service was done by the same provider. It goes on the E/M code.
Modifier 59 is used when there are multiple procedures that are independent from one another performed on the same DOS by the same provider. The XE, XP, XS, XU modifiers offer more specificity than the 59 modifier.
An excision is when the physician removes only a portion of a body part without replacement. An example would be the physician removing a portion of the sigmoid colon.
A resection is when the physician takes out all of a body part without replacement. An example would be the physician removing the entire gallbladder (cholecystectomy).
