In the state of Georgia every single one of our payers has denied an office visit billed same day as ETT, 93015. I was informed that it is OK to append a -25 modifier on the ov if the testing was a result of the OV. If the test came before the OV, we are not allowed to append a modifier, unless there are changes. What would you consider a change? Is changing medicine for the patient a change, giving cardiac clearance, ordering labs, etc..? Please shed some light on this for me. I am very concerned that we might be doing this incorrectly or that we will lose money if we don't append the modifier.
