When adding modifier 22-to an appy for extensive lysis of adhesions as an example--should the fee of the procedure code be increased to represent the additional work? If so, what percent?
 
Usually ...

First, we require that our surgeons specify the amount of time required for the "extensive lysis of adhesions" ... If it is 1 hour or more then we add the -22 modifier to the primary procedure.

We increase the primary procedure fee by 25%.
