When the psychotherapy visit is 60 minutes long, do I code this to 90806 (approx 45-50 minutes) with modifier 22 (increased services) OR 90808 (approx 75-80 minutes) with modifier 52 (decreased services)? I can't find specific guidance anywhere. I've always employed the thought that you use the 90806 because the provider spent more than 50 minutes with the patient. I'm being advised to code up to the higher time limit. What do you do in this situation????
