Would it be ok to add modifier 22 to an op note for lysing adhesions if only 20 extra minutes was spent on the procedure? What is a good amount time that would justify the 22?
 
To me, 20 minutes does not justify -22 on a major surgical procedure.
I have seen guidance varying from 30 minutes minimum up to 60 minutes minimum. In our cases, we use 30 minutes of lysis as the threshold for -22. I request additional 20% payment for 30 minutes; additional 30% for 45 minutes; additional 40% for 60 minutes, etc.. Many other practices will use the 60 minute minimum.
