Wiki Modifier 24 & 57 - decision for surgery

When billing an E&M visit, is it acceptable to use both modifier 24 & 57 if the E&M is within a post op period but also a decision for surgery was made?

sounds like a 79 modifier would be required as well then?

Well, the 79 would go on a procedure. I'm just inquiring about the E&M visit itself. Any thoughts??

If related PDx, E/M is not billable.

If unrelated PDx, append mod 24 & 57 to appropriate E/M level.
