Modifier 25 day of chemo

Are your docs charging for OV on day of chemo with modifier 25? If not, how are you scheudling your patients for OV and chemo - seperate days? How often? thanks
 
My physicians also see patients on the same day of chemo. In order the bill correctly the office visit must be a separate identifiable service apart from the chemotherapy and then you can use the 25 modifier.
