Wiki Modifier 25? Office and px same day

I patient came in w/DX for hemorrhoid's and then the doctor did a IRC 46390. The DX is the same on the E/M and indications of the procedure. I was told you can't do both w/the 25 if the dx is the same on the office visit and in the indication of the op report. Is this correct?

Do you mean 46930? You can use modifier 57 for the E/M if the decision of surgery is made prior to the surgery on the same day.
