Wiki Modifier -25 on sick visit or well visit?

If I have a sick visit and a well visit, does it matter on which cpt code the modifier -25 needs to be on?:confused:
 
I am assuming the patient was brought in originally for a a well visit so I would append the -25 modifier to the sick visit.
