Is Modifier 25 required on an E/M code when tests are performed on the same DOS? Test would be 81003 UA dip, 87804 Flu screen, 87811 Visual Covid screen, basically any code that would start with 8?

as far as i know for lab test with E & M code , do not need to add modifier 25.

Typically labs do not require the E/M to have a modifier 25. I have never seen this, and in fact have seen denials from insurance when other coders have mistakenly appended a 25 modifier to the E/M with labs. Be sure to include a QW to your 81003 if it qualifies.
