I was recently told that you had to have a seperate note to use the modifier 25?? My coworker and myself agree that as long as the work is documented there is no need for a seperate note. I've done some research and I have not found anything that says a seperate note is necessary but I wanted a second opinion.
Any advice would be greatly appreciated.
 
I have always found it necessary to have the procedure documented as a separate paragraph in the note. Otherwise the carrier informs me that the elements of the 25 modifier has not been met.
 
As long as there is a documented description of the procedure by the same physician that performed the E&M you should be able to use modifier 25 with the E&M code.:)
