Pt came into office on friday 1-21-11. Physician scheduled a LHC to be performed on 1-24-11, should or can I append -57 to the office visit?:confused:
 
No, considering heart caths have a 0 day global you wouldn't append a -57 modifier.

-57 can only be used on e/m's for procedures that have a 90 day global.

You won't need any modifier on that particular visit.

Always take into consideration the global periods that apply to procedures and the situation under which "the decision for surgery" was made.
 
57 is only used on the day of or day before a 90 day global. Any other global requires no modifier the days before and only a 25 the day of if the E&M is more extensive or for a different reason.
