Can you use the 57 if the decison was made for surgery at the EM visit but surgery will not be done for 2 weeks or so? It would be for a total hip or total knee surgery. Thanks so much
 
you wouldn't want to, would you? there's no need for the modifier for "decision for surgery" if it isn't within the global period of the procedure (day of day before surgery). Two weeks down the line, does not require the modifiers' use...
