procedure: 2nd surgery--removal of packing of left dorsal wrist and hand wound with repeat irrigation of skin, subcutaneous tissue and muscle.

1st procedure: 25028, I&D of forearm/wrist

Looking at 11010 with modifier 58

Not sure if modifier 58 would be more appropriate than 78.

Sounds like it was a planned procedure. I'd use modifier 58.

11010 involves debridement of an open fracture or dislocation.

Without the medical record, it sounds like this packing/dressing change is included in the 25028 global.

I agree with MOJO, without the operative notes, and based solely on the information provided, the second procedure is inclusive in the global.
