Wiki Modifier 66 Coding Set???

I wanted to check if we can use modifier 66 with cpt code 43361. If yes, please provide with a link for related info.

it looks like it allows a co-surgeon or an assist but not a whole team

There were 4 surgeons that performed this procedure and Medicare denied the claim. They will allow modifier 62 but there were more than 2 surgeons who operated on the patient. How should I bill?

I think those other 2 physicians are out of luck unless they performed some completely different procedure.
