Wiki Modifier 76 input

The practice I work for has been using modifier 76 on the third level of injections. I am pretty sure this is incorrect. I'm under the impression this modifier is only when the physician repeats the same procedure, same level for some reason or another not so much when the Dr. injects into the next level.

Can anyone give me some input please?
 
That would really depend on the insurance in question...

I know BCBS requires mod -76 for more than one injection while others require mod -59. To me, that's like mod -50... Some insurances will accept and some will not. :-/

I haven't had an issue using -76 for BCBS when coding for mulit-level injections and -59 for other insurances.
