If a physican has cath placement in 4 vessels 3rd order and you have codes
36247, 36248, 36248,36248 for the cath placement. is it correct to apply the 76 modifier to TWO of the 36248 codes? and would that apply with the S&I codes 75736, 75774, 75774...would you apply the 76 to one of the 75774's?

the code 36248 is done correct, but the procedure is done on different artery, which mean it is distinct from the previous one, if 36248 is done on first artery, the other 36248 is done on second artery, so to differentiate or distinct it form the other one we use 59 modifier not 76. :)
