As per medical records , CPT 73721 was performed by same provider in same dos , Firstly on knee and then on hip joint , so the two units of CPT 73721 will be billed with different diagnosis (Knee and Hip ) , But kindly guide , Either Modifier 76 (Repeat procedure ))should be appended with the second unit or Modifier 59 ( XS-Separate structure)should be appended. ???

It would be the Modifier 59 or the X modifier.

Modifier 76 is for same day, same body part, same physician.
