can someone help with modifier 76 correct usage. if the patient was seen in office and had bilateral thumb injections and bilateral index finger injections at the same encounter would 76 be appropriate on the second line of the claim. (I.e 20550-50, 20550-50,76)?

I do not think -76 would be correct here. You are not repeating the procedure, you are doing the same procedure on another digit.
I am not an ortho coder, but I believe the correct way to code this is 20550-50 & 20550-50-59. I think you also need the modifiers for the individual digits, FA, F5, F1, F6.
