If a patient has a colostomy performed & then 10 days later a revision of the colostomy is performed, is the revision billed with mod 78?
 
I believe you should use modifier -76 ( Repeat Procedure or Service by same physician: It may be necessary to to indicate that a procedure was repeated subsequent to the original procedure).:)
 
I would say the revision probably needs a -78, unless the first op note mentions a plan to return to the O.R. later to revise something, in which case it would take the -58.
