Your opinion would be greatly appreciated:

The patient had a major surgery 90 day global period. After 2 weeks the patiet has an infection at the site of the surgery. He returns to the MD's office for a debridement of the wound. Can this procedure be billed with modifier 78? My concern here is that the patient did not return to the hospital and the treatment of the wound was done in the office.

Any payor that follows Medicare global guidelines will not be billable. Complications that do not go back to the OR are considered part of the surgical package.

Check the payor rules before deciding.
