Wiki Modifier 79-different procedure

We had an office visit and did a totally different procedure during a global period. I know that I need to add the modifier 79 but do I add it to just the offive visit code or just the procedure code or both? Thanks for you help.
 
If you have an ov that is unrelated to the global you need a 24, if that same ov is significant to the procedure then performed you also need a 25, then for the procedure you need the 79.
