Pt had procedure 17000 on 03.11.2012 and came back on 03.17.2012 for same procedure 17000. We submitted claim for 03.17.2012 with modifier 79. Now it has been rejected by UHC for incorrect modifier.

Please correct me if this was billed wrong. Thanks!
 
You may want to try mod 58. This is for a staged procedure that is either planned or not.
Since the procedure was done on the same lesion this would be appropriate as a more extensive treatment was needed.
