i see there has been some discussion regarding modifier 80 however i have not seen anything newer than 2016.. my question is whether a "statement" is needed by the main surgeon in order to append a modifier 80.
1. is a statement needed?
2. do you list the procedure twice and one with modifier 80?
3. when the assistant is a PA is AS used and is a statement needed as well?
when i say statement i read a few articles that a statement consists of; a resident wasn't available; medical circmstances required an assistant ... ? is this correct?
I have googled and read the CPT book but I'm either missing something or it's not there .. I appreciate any and all help. Thank you.
