Wiki Modifier 91

I am verifying CPT/HCPC's codes for Wound Clinics. One clinic is billing for the blood glucose monitoring done at the time of HBO treatment with a modifier 91 and one clinic is billing the same test without the modifier. I am trying to find out which is correct.
