I was wondering if anyone can help. I have a patinet that came in for a I & D of a Pilonidal cyst. The patient then went to see a surgeon that is now going to do surgery on it. The patient came back to us to have a pre-op physical 3 days later. This is within the global period. Can we bill for the pre-op? Is there a modifier I have can use? I don't find any of the modifiers appropriate for this situation. I have a feeling it is with the global and we will have to eat the pre-op physical. Thoughts? Thank you!
 
Probably a 10 day global if a code from 10080 range was done prior.

I think it depends on the documentation and the extent of the examination done at the pre-op physical. It would be a 24 modifier on the E/M if the E/M meets requirements to be coded. If you threw out anything related to the procedure done in the 10 day global, is there enough left to code an E/M? There could be additional factors that have to be considered if the patient is undergoing general anesthesia possibly and depending on the complexity of the surgery. Also, if the patient has other conditions to be taken into consideration that could complicate things. If it was very simple and nothing "new" was done and it pretty much mirrored the prior E/M 3 days prior (if an E/M was billed with the 1008_) I probably would not separately bill.
